California Can’t Keep the Lights On but Gavin Newsom Found the Budget for Illegal Alien Boob Jobs — Your Tax Dollars at Work

A new watchdog report from the Manhattan Institute just dropped, and folks, even by California standards this one is spectacular. It turns out that Gavin Newsom’s state government has been spending taxpayer money on breast implants, sex-change operations, and cross-sex hormone therapy — for illegal aliens. Not legal residents. Not citizens. People who aren’t supposed to be in the country.

The horror! California can’t figure out how to keep the power grid running in August or stop people from pooping on the sidewalks in San Francisco, but they’ve carved out a nice little budget line for cosmetic surgery on people who hopped the border. Priorities!

Here’s how this works. Back in January 2024, the “hair apparent” Newsom expanded Medi-Cal — that’s California’s Medicaid program — to cover all immigrants regardless of legal status. Every single one. Show up, sign up, get your “gender-affirming care” on the house. And by “on the house,” we mean on the backs of California taxpayers who are already drowning in a $21 billion budget deficit headed into 2027.

Chris Rufo at the Manhattan Institute dug into the numbers and found that 1.7 million illegal immigrants received full-scope Medi-Cal coverage. The total price tag for healthcare for illegal immigrants? Nine billion dollars. That’s billion with a B.

And what are Californians getting for that nine billion? Well, among other things, a transgender woman from Honduras is getting cross-sex hormone therapy on your dime. Another migrant walked in and got taxpayer-funded breast implants. Not reconstructive surgery after an accident. Cosmetic breast implants. Because apparently that’s what “medically necessary” means in Newsom’s California.

The White House didn’t mince words. Steven Cheung, the Communications Director, called it “the holy trinity of woke, liberal, out-of-touch, perverse ideology from Scumbag Gavin Newsom.” We couldn’t have said it better ourselves. (Actually, we probably could have, but his version is pretty solid.)

Now, Newsom’s office fired back with one of those beautifully tone-deaf statements that only Sacramento could produce: “Undocumented Californians don’t get special treatment. Everyone on Medi-Cal gets the same access to care.” Oh, well that makes it totally fine then! We’re not giving illegal aliens SPECIAL cosmetic surgery — we’re giving them the SAME cosmetic surgery that every other person on welfare gets. Pack it up, boys. Nothing to see here.

The California Department of Health Care Services chimed in too, claiming that Medi-Cal only covers gender-affirming care “when it is medically necessary.” Medically necessary. Breast implants for an illegal alien are medically necessary. That’s the official position of the state of California. Somewhere, a veteran is waiting nine months for a knee replacement at the VA, but sure, let’s fast-track the boob jobs.

Here’s the kicker — California actually had to PAUSE new Medi-Cal enrollment in January 2026 for adults over 19 without legal status. Why? Because the costs got so out of control that even Sacramento couldn’t pretend the math worked anymore. They spent themselves into a hole so deep they had to slam the brakes. But the damage was already done. Nine billion dollars, gone.

This is what Democrats want for your country. This isn’t some fringe proposal from a city council member in Portland. This is the sitting governor of the largest state in America, the guy who thinks he should be president someday, spending billions of dollars providing cosmetic surgery to people who broke the law to get here. And when he got caught, his defense was “everyone gets it, not just the illegals.” As if that makes it better!

Meanwhile, California families are fleeing the state in record numbers because they can’t afford housing, can’t afford groceries, and can’t walk down the street without stepping over a tent city. But at least the illegal aliens in those taxpayer-funded homeless shelters in San Francisco are getting their hormone treatments on schedule.

Gavin Newsom looked at a $21 billion budget deficit and thought, “You know what this state really needs?
